Spravato® Treatment

For depression that hasn’t responded to other treatments — delivered safely, in our office, by a REMS-certified provider.

Spravato® (esketamine) is an FDA-approved nasal spray for adults whose depression hasn’t improved with traditional antidepressants. It works on a different pathway in the brain — and for many patients, it offers relief when other treatments haven’t.

Because Spravato carries specific safety requirements, it’s only delivered in certified clinics under direct medical supervision. Dr. Sarah Graff is REMS-certified to provide it, and treatment is closely monitored from start to finish.

Who Spravato is for

Spravato is FDA-approved for two specific situations: treatment-resistant depression, and major depressive disorder with active suicidal thoughts or behavior. You may be a candidate if you:

  • Have tried two or more antidepressants without enough improvement
  • Are living with persistent, severe depression
  • Have suicidal thoughts that haven’t lifted with current treatment

A referral is required

Spravato treatment requires a referral from your primary care provider or psychiatrist. We’ll coordinate with your referring provider throughout treatment and screen carefully to confirm Spravato is the right fit before scheduling.

REMS-certified care

Dr. Graff is certified through the Spravato Risk Evaluation and Mitigation Strategy (REMS) program — the federal safety framework that governs how Spravato is dispensed and monitored. It means your care follows strict protocols designed to keep you safe.

A few important notes

  • Treatment is in-office only — not available by telehealth
  • Patients cannot drive after a session and need a ride home
  • Plan for about 2.5 hours per visit

What a treatment session looks like

Clinical check-in

We take your vital signs and check in on how you’ve been doing since your last visit.

Self-administered dose

You take the nasal spray yourself, with our team in the room and ready to support you.

Two-hour observation

You rest in a quiet, comfortable room while we monitor how you’re responding — required by REMS protocol.

Discharge home

Once we’re confident you’re safe to leave, your ride takes you home. You won’t drive yourself.

Insurance and prior authorization

We accept most major insurances and are in-network with the Emory Healthcare Network. Spravato typically requires prior authorization, and most commercial plans cover it once that’s in place. We handle the paperwork and verify your benefits before scheduling so there are no surprises.
